What to bring with you

• Complete list of all medications you are currently taking (include name, dosage & directions)
• Copies of office notes and x-rays pertaining to your current medical condition (if applicable)
• Most recent insurance card as well as the insured’s name, date of birth, and employer
• Accurate billing address, telephone #, and other relevant information
• Care Givers: patient’s address, telephone #, Social Security #, billing information
• Worker's Compensation: date of accident, copy of accident report, billing address, name/telephone # of contact person

Facts to be aware of

• Date (month, year) of any past hospitalizations and/or surgeries
• Medicare Patients: name of your medical doctor and the date of your last visit with him/her
